Radiation Techniques

Advanced radiation oncology,
matched to the cancer.

Radiation therapy can be used before surgery, after surgery, alongside systemic treatment, or as part of the primary treatment for selected cancers. The right technique depends on the diagnosis, the tumour and the individual patient.

Proton Therapy

Protons release their energy at the tumour and stop, reducing the dose to normal organs — used for selected head and neck, skull base, breast and gynaecological cancers.

IMRT

Intensity-Modulated Radiation Therapy shapes and modulates the dose to the treatment target. Dr. Nangia helped establish one of India’s early IMRT programmes.

IGRT

Image-Guided Radiation Therapy uses imaging during treatment for accurate positioning, and daily IGRT reduces the volume of normal tissue that receives radiation.

SBRT

Highly focused radiation in a few sessions for oligometastatic disease — a limited number of liver, lung, bone or lymph nodal metastases.

Helical Tomotherapy

Image guidance with highly modulated delivery: less dry mouth and swallowing difficulty in head and neck cancers, and bowel and bladder sparing in gynaecological cancers.

Brachytherapy

Internal radiation placed close to or within the treatment area, including advanced image-guided brachytherapy.

About

Meet Dr. Sapna Nangia.

Senior Radiation Oncology Specialist

Dr. Sapna Nangia brings 40 years of clinical experience and nearly 35 years in oncology. She graduated from the Armed Forces Medical College (AFMC), Pune, served for five years in the Army Medical Corps of the Indian Army, and completed her MD in Radiotherapy at Sanjay Gandhi Post Graduate Institute (SGPGI), Lucknow.

During her tenure at Batra Hospital she established one of India’s early IMRT programmes in 2002–2003, and led pioneering clinical work and important publications on head and neck IMRT. In 2009 she established radiation oncology services at Fortis Hospital, Noida — the first for the Fortis and IOSPL groups — and introduced IGRT and SBRT.

At Indraprastha Apollo Hospital, New Delhi, from 2012 to 2019, she led the modernisation of the radiation department, introduced MRI-based brachytherapy, developed memory-preserving radiation approaches for cheek cancers, and invented and patented a deep-inspiration breath-hold (DIBH) device for heart-sparing radiation in breast cancer.

Since 2019 she has led the proton therapy programme for head, neck, skull base, breast and gynaecological cancers at Apollo Proton Cancer Centre, Chennai — the first proton therapy centre in this part of Asia. Her work focuses on precise delivery designed to improve outcomes and reduce adverse effects, and includes inoperable head and neck, recurrent and complex skull base cancers, where proton therapy has advantages over X-rays.

She is also actively involved in clinical research, publications, teaching and the mentoring of postgraduates and registrars.

  • Post-mastectomy radiotherapy
  • Reduced dose to the lungs and opposite breast
  • Lower second-cancer risk in younger women
  • Patients with diabetes, hypertension or cardiotoxic chemotherapy

Gynaecological Cancers

Tomotherapy and image-guided brachytherapy

Inoperable, recurrent and complex cancers

Inoperable head and neck cancers, recurrent disease and complex skull base tumours are difficult to treat, and proton therapy has advantages over X-rays in these clinical situations. Send your reports for an opinion before you travel.

Discuss Your Case
Proton Therapy

The dose stops where
the tumour stops.

Proton therapy uses protons to deliver radiation to selected tumours. A conventional X-ray beam keeps depositing dose on its way out of the body; a proton beam travels to a set depth, releases most of its energy there, and stops.

Its advantages include a proven reduction in acute side effects at certain disease sites, improved outcomes in various cancers, and a reduced dose to normal, unaffected organs.

Lower dose to the heart, its vessels and muscle in breast cancer — especially with diabetes, hypertension, obesity or cardiotoxic chemotherapy
Improved outcomes in skull base cancers beside the brainstem and the pathways for vision, memory and hearing
Less dose to the lungs, opposite breast and flank reducing the odds, though low, of radiation-induced second cancers in young women
A proven reduction in acute side effects at certain disease sites
An option in inoperable and recurrent disease in head and neck and complex skull base cancers
Am I Suitable for Proton Therapy?
Conventional X-Rays

Dose is deposited before, through and beyond the target — the tissue on the far side is irradiated too.

Proton Beam Therapy

Energy peaks at the tumour and falls to zero immediately after — the tissue behind it receives almost nothing.

Proton therapy is suitable only for selected patients. Cancer type, tumour location, previous treatment and the objective of treatment all decide it — which is exactly what a second opinion is for.

Personalised Planning

Your treatment should be
planned around you.

Selecting a radiation treatment is not simply about choosing a technology. The appropriate approach depends on the individual patient’s cancer — which is what the first consultation is for.

01

Send your reports

Email your biopsy and histopathology report, recent CT, MRI or PET imaging and a summary of treatment already given — or call the clinic and we will guide you on what to send.

02

Personal assessment

Dr. Nangia reviews the diagnosis and pathology, the tumour type, stage, location and extent, your imaging, previous treatments and any planned surgery or systemic treatment.

03

Technique and plan

That assessment decides whether radiation therapy or proton therapy is appropriate and which technique suits your cancer, followed by immobilisation, planning scans and personal contouring.

04

Treatment & follow-up

Daily treatment with weekly review, supportive care through the course — nutrition, physiotherapy and swallowing therapy where needed — and structured follow-up afterwards.

Get in touch

Dr. Sapna Nangia is a senior Radiation Oncologist and Proton Therapy Specialist in India with 40 years of clinical experience, including nearly 35 years in oncology. Her current focus is head and neck cancers, skull base cancers, breast cancers and gynaecological cancers, at Apollo Proton Cancer Centre, Chennai.

Her current clinical focus includes head and neck cancers, skull base and sinonasal cancers, breast cancers and gynaecological cancers.

Her expertise includes Proton Therapy, IGRT, IMRT, SBRT, Helical Tomotherapy and Brachytherapy, including image-guided brachytherapy and a patented deep-inspiration breath-hold (DIBH) workflow for heart-sparing breast radiation.

No. Proton therapy is suitable only for selected patients, depending on factors such as the cancer type, tumour location, treatment objectives and individual clinical considerations. Where it offers no advantage, a well-planned IMRT, IGRT or tomotherapy course is recommended instead.

The number of sessions depends on the cancer type, the objective of treatment, the radiation technique and the individual treatment plan. SBRT, for example, is delivered in a few sessions, while many head and neck courses run over several weeks.

A radiation oncologist may be consulted after a cancer diagnosis, before or after surgery, when radiation therapy has been recommended, or when proton therapy is being considered.

Email your reports to consult@drsapnanangia.com or send them on WhatsApp to +91 98910 10390. Most patients get an initial view without travelling. If a consultation is needed, the clinic will help you plan a single trip that covers the review, the planning scan and the start of treatment.

At minimum: the biopsy / histopathology report, the most recent imaging (PET-CT, MRI or CT — images on a CD or DICOM link are far more useful than the report alone), any surgical notes, and a summary of chemotherapy or radiation already received, including dose and dates if available.

Often, yes — but it depends on the dose already delivered, how much time has passed, and which structures sit in the field. Proton therapy makes re-irradiation feasible in a number of head, neck and skull base recurrences that cannot safely be re-treated with photons. Every such case is assessed individually, and a clear no is given where a second course would do more harm than good.

Most head & neck and skull base courses run five days a week for six to seven weeks; many breast schedules are considerably shorter. Treatment itself takes minutes a day, so patients stay locally rather than in hospital. The clinic team can advise on accommodation near the centre for you and one attendant.
